1) Download the form here - Paramount Work Authorization
2) Print and Complete the information located at the top of the form
3) Initial the authorization allowing Paramount Centre to endorse the insurance check
4) sign and date the form at the bottom
5) Fax it to Paramount Centre at 253.896.1333
